In all sincerity though, this idea is bullcrap. First of all, I would probably laugh my ass off if I read the above disclaimer on an actual suicide note (which would be highly inappropriate). And then there's the fact that the BSD license is for use on software. "Source code" and "binary forms" hardly applies to a suicide note, unless you write it in markdown and compile it to HTML, maybe (if you really consider doing that instead of using a pen and paper, you probably _should_ kill yourself).
I'd say Creative Commons would be more suited for this kind of work.
